Primary and secondary neoplasms

Distinguishing a primary site from a metastatic deposit is the central decision in neoplasm coding.

Code the primary site and each secondary site separately. A liver lesion is far more often a secondary deposit (C78.7) than a primary hepatocellular carcinoma (C22.0) - the pathology report decides.
Multiple independent primaries each receive their own code; a neoplasm extending into adjacent tissue does not.
Where a lesion overlaps contiguous subsites and the origin cannot be determined, use the .8 overlapping-lesion code.
