Acute and chronic conditions
When both an acute and a chronic form of the same condition are documented and separate codes exist, both are assigned.

- Where the classification provides a single combination code covering both, use it instead of two codes.
- Acute kidney injury superimposed on chronic kidney disease is the canonical example - both are coded.
